  3717-1-06.1 Physical facilities: design, construction, and  installation.
  (A)Floors, walls, and ceilings.
  Except as specified under paragraph (D) of this rule and except  for antislip floor coverings or applications that
  may be used for safety reasons, floors, floor coverings, walls,  wall coverings, and ceilings shall be designed,
  constructed, and installed so they are smooth and easily  cleanable.
   
   3717-1-06.4 Physical facilities: maintenance and operation.
  (A)Repairing.
  The physical facilities shall be maintained in good repair.

  VIOLATION: Broken light cover in dishwashing area.
   
  3717-1-06.1 Physical facilities: design, construction, and installation.
  (I)Light bulbs - protective shielding.
      (1) Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food;
      clean equipment, utensils, or linens; or unwrapped single-service or single-use articles. This paragraph does 
      not apply in areas used only for storing food in unopened packages, if the integrity of the packages can not be
       affected by broken glass falling onto them; and the packages are capable of being cleaSeal up eny gaps in the building
      that might allow the pests inside. ned of debris from broken

  Violation: Floors, especially in the back room and at wall floor junctures, need to be cleaned. .
   
  3717-1-06.4 Physical facilities: maintenance and operation.
  (B)Cleaning - frequency and restrictions.
      (1) The physical facilities shall be cleaned as often as necessary to keep them clean.
      (2) Cleaning shall be done during periods when the least amount of food is exposed such as after closing. 
          This requirement does not apply to cleaning that is necessary due to a spill or other accident.

  Violation: Handsink and the three compartment sink are in need of cleaning. A deliming agent might be needed to deal with
  mineral accumulations.

  (H)Cleaning and maintenance of plumbing fixtures.
  Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary 
  to keep them clean and maintained.

  Violation: Evidence of mouse infestation at the coffee counter and near coolers. Currently, the owner is doing his own pest
  control using glue traps. A licensed Pest Control company must be employed. Clean up the mouse waste daily
   
  3717-1-06.4 Physical facilities: maintenance and operation.
  (K)Controlling pests.
  The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises
  by:
      (1) Routinely inspecting incoming shipments of food and supplies;
      (2) Routinely inspecting the premises for evidence of pests;
      (3) Using methods, if pests are found, such as trapping devices or other means of pest control as specified
      under paragraphs (C), (K) and (L) of rule 3717-1-07.1 of the Administrative Code; and
      (4) Eliminating harborage conditions.

  VIolation: Equipment that is not used and will not be used needs to be removed. The slushie machine on the counter going
  unused is growing mold inside and is in the way for cleaning up after the mice.

  (N)Maintaining premises - unnecessary items and litter.
  The premises shall be free of:
      (1) Items that are unnecessary to the operation or maintenance of the food service operation or retail food
      establishment such as equipment that is nonfunctional or no longer used; and
      (2) Litter.

  Temperature in coolers are at or under 41°F.
